Student Forms (Beginning of School Year & Athletic Forms)
We will be conducting the Back to School form completion and collection process through Scribbles. Every family in the district will be asked to create a Scribbles account and complete a renewal application in which they will provide updated contact information. In addition, families will be asked to complete a number of forms that relate to back to school. Please use the link below and follow the listed instructions to create your family account and complete the required forms and update any information.
Please complete the following steps to complete the back to school process:
- Create a Scribbles account
- Associate each of your students to your Scribbles account
- Complete the Back to School application for each of your students
- Complete the requested forms for each student and return to the school
To streamline the process of obtaining athletic forms, we have placed the Concussion, Physical and Eligibility forms in Scribbles for all Middle and High School parents to download, print, complete and return to the school and/or coach. Furthermore, the local athletic forms (Parent and Student Pledge, Parent Permission and Emergency Contact form) have been created in Scribbles for you and your student to complete online and submit. Please use the link below and follow the listed instructions to complete the required athletic forms.

- Create a Scribbles account
- Associate each of your students to your Scribbles account
- Download the athletic forms from the family dashboard to be completed and returned to the school/coach
- Complete and submit the online athletic form
